A Letter from P'Fella
5 Ideas for a Journal Partnership
I’m super excited to let you know that we’re looking into a potential partnership with some big journals to make evidence-based content way more accessible for all of you. But before we jump in, I really want your thoughts—your input is what will make this work best for everyone.
From the feedback we’ve gotten, it’s pretty clear that some of the biggest barriers to using plastic surgery journals are paywalls, too many logins, lack of concise content, and sometimes, relevance to what you actually need day-to-day.
These are real challenges, and we want to find solutions that really work for you. Imagine if getting high-quality, evidence-based info was simple, easy, and actually useful to your practice.
How would you prefer to access more evidence-based content through our journal partnership?
Your voice is key to making this work. Your feedback will help us create something that’s actually helpful and makes keeping up-to-date easier and more impactful.
Let’s make this happen together.
With love,
P'Fella ❤️

Reverse Anterolateral Thigh Flap
In this section, we feature an anatomical illustration. This edition looks at a reconstruction option for the lower limbs: Reverse anterolateral thigh flap.
Technique Tip
Bilobed Flaps
This edition looks at Zitelli's geometric bilobed flap design. Read our article for an in-depth look at the bilobed flap technique and its applications.

28 Days in Gaza: A Conversation with Ms Victoria Rose
In our upcoming episode, we sit down with Ms. Victoria Rose to discuss her experiences as a plastic surgeon in Gaza. What’s it like to handle complex trauma and reconstructive cases in such challenging conditions? How do surgeons manage under intense pressure and with limited resources?
Tune in as we address these questions and more, diving into the realities of frontline surgical care.

What Does the Evidence Say?
The Various Modifications of the Scalp Rotation Flap
Traditionally, flaps are planned to be up to 8 times the defect diameter, but modified techniques like the "snail flap" can use smaller ratios (Christopoulos et al., 2020). Personalized computational models can predict mechanical stress and tension distribution in scalp flaps, potentially improving surgical planning (Lee et al., 2020).
Various modifications, such as double-opposing unilobar rotation flaps, have been developed to address larger defects (Moulton-Barrett & Vanderschelden, 2015). Experimental studies have explored optimal flap designs, including the use of tissue expanders (Vendroux et al., 1977; Lo & Kimble, 2008). Geometric considerations are essential for successful reconstruction, with transposition flaps being a reliable option for full-thickness scalp defects (Ahuja, 1988; Shah et al., 2022).

The fasciocutaneous flap offers reliable vascular support for lower leg soft tissue reconstruction, enabling large, stable flaps with straightforward postoperative management and compatibility.
The bilobed flap is a double transposition flap ideal for covering defects on the lower third of the nose, addressing both functional and aesthetic needs with reduced risks of tissue protrusion and "pincushioning".
The chondrocutaneous advancement flap technique combines cartilage and skin advancement to restore form and function effectively in cases of ear deformities.
